What is Clinical Case Management?

  • Provide weekly support from a Case Manager

  • Develop individualized service plans and goals

  • Provide community resources, information and connections

  • Assist with systems navigation

  • Reduce barriers to care and recovery

  • Teach skills building

  • Support independence, housing and recovery
